Wufoo is one of the oldest products in this category still under active development, and it behaves like it — the editor is fast, the report views are thorough, and nothing about it is trying to be fashionable.
The structure is a fixed set of field types, rules that show and hide them, and a built-in reporting layer that most newer tools leave to a spreadsheet. Payment fields are included, and entries are managed inside the product rather than exported to be useful.
The cost is the shape of the plans. The free tier stops at five forms, a hundred entries and ten fields, which is a demonstration rather than a plan, and every paid tier charges five cents for each entry over its monthly allowance — so an unexpectedly popular form produces an unexpected invoice rather than a hard stop.
